'Very serious dereliction of duty' behind fatal pipeline blast

The deadly oil pipeline blasts in Shandong Province's Qingdao City?on Friday exposed three severe problems of human errors, and the accident was a "very serious dereliction of duty", Yang Dongliang, director of the State Administration of Work Safety, said on Monday.
The problems include the unreasonable layout of oil pipelines and urban drainage pipes, negligence of duty on pipeline supervision, and unprofessional handling of oil leakage before the blasts, said Yang.

He made the remarks in Qingdao at a meeting of an investigation team for the accident, which has left 55 people dead, nine missing and 136 hospitalized as of Monday morning.
Yang requested that the investigation team speed up the investigation and publicize the report in a timely manner.
According to him, the State Council has invited staff of the Supreme People's Procuratorate to join the investigation.
Chinese President Xi Jinping has ordered a large-scale production safety check and warned various industries to learn the lesson after Friday's fatal explosions from a leaking pipeline.
He also urged all companies to pay adequate attention to safety investments, safety training, grassroots management and emergency rescue and relief under the strict supervision from governments at all levels.
